Diabetes Care: Supporting You to Better Manage Your Diabetes

I have a particular interest in diabetes care and have completed an Advanced Diploma in Diabetes Care, which has given me further knowledge and skills in supporting people living with diabetes.

Diabetes is a condition that requires ongoing care and, in my experience, good diabetes management is about much more than simply looking at your blood sugar levels. I enjoy working with patients to understand their individual circumstances, identify areas where we can make positive changes, and develop a management plan that feels achievable and sustainable.

Understanding Your Diabetes

Whether you have recently been diagnosed with diabetes or have been living with it for many years, regular review is an important part of your care. I can help you understand your diabetes, review your blood glucose levels and medications, discuss lifestyle factors and identify areas where your management may need to be adjusted.

For patients with an ongoing health condition, a Chronic Condition Care Plan may also be appropriate. These plans provide a structured approach to managing your health and can help coordinate your care with other healthcare professionals where needed.

Convenient On-Site Pathology

Clinipath Pathology is available onsite at Health by Nurture, making it convenient to have blood tests and other pathology investigations completed as part of your ongoing diabetes care.

I believe diabetes care should be personalised. Everyone's circumstances, goals and challenges are different, so I take the time to understand what is important to you and work with you to develop a plan that suits your needs.

My aim is to help you feel informed and confident in managing your diabetes, while supporting you to make small, sustainable changes that can have a meaningful impact on your long-term health.
